The editors of the JRE solicited short essays on the COVID‐19 pandemic from a group of scholars of religious ethics that reflected on how the field might help them make sense of the complex religious, cultural, ethical, and political implications of the pandemic, and on how the pandemic might shape the future of religious ethics.

Although informed consent is important in clinical research, questions persist regarding when it is necessary, what it requires, and how it should be obtained. The standard view in research ethics is that the function of informed consent is to respect individual autonomy. However, consent processes are multidimensional and serve other ethical functions as well. These functions deserve particular attention when barriers to consent exist. We argue that consent serves seven ethically important and conceptually distinct functions. The first four functions pertain (...) principally to individual participants: (1) providing transparency; (2) allowing control and authorization; (3) promoting concordance with participants' values; and (4) protecting and promoting welfare interests. Three other functions are systemic or policy focused: (5) promoting trust; (6) satisfying regulatory requirements; and (7) promoting integrity in research. Reframing consent around these functions can guide approaches to consent that are context sensitive and that maximize achievable goals. (shrink)

We estimate that 208,000 deep brain stimulation (DBS) devices have been implanted to address neurological and neuropsychiatric disorders worldwide. DBS Think Tank presenters pooled data and determined that DBS expanded in its scope and has been applied to multiple brain disorders in an effort to modulate neural circuitry. The DBS Think Tank was founded in 2012 providing a space where clinicians, engineers, researchers from industry and academia discuss current and emerging DBS technologies and logistical and ethical issues facing the field. (...) The emphasis is on cutting edge research and collaboration aimed to advance the DBS field. The Eighth Annual DBS Think Tank was held virtually on September 1 and 2, 2020 (Zoom Video Communications) due to restrictions related to the COVID-19 pandemic. The meeting focused on advances in: (1) optogenetics as a tool for comprehending neurobiology of diseases and on optogenetically-inspired DBS, (2) cutting edge of emerging DBS technologies, (3) ethical issues affecting DBS research and access to care, (4) neuromodulatory approaches for depression, (5) advancing novel hardware, software and imaging methodologies, (6) use of neurophysiological signals in adaptive neurostimulation, and (7) use of more advanced technologies to improve DBS clinical outcomes. There were 178 attendees who participated in a DBS Think Tank survey, which revealed the expansion of DBS into several indications such as obesity, post-traumatic stress disorder, addiction and Alzheimer’s disease. This study explores linkages between what Chinese managers generally know about environmental issues, how strongly they value environmental protection, and different types of behaviours/actions they may take within their organizations on behalf of the environment. From a sample of 305 managers in Guangzhou and Beijing, it was found that both environmental knowledge and values are more predictive of more personal managerial behaviours, such as keeping informed of relevant company issues and working within the system to minimize environmental impacts, than more (...) overt behaviours. Moreover, for these more personal actions, environmental knowledge and values were found to have both main and interactive effects. By comparison, it was found that both environmental values and knowledge had additive effects on managerial tendencies to initiate new programs within their domain of responsibility. Objectives: To study the attitudes of both medical and non-medical students towards the do-not-resuscitate decision in a university in Hong Kong, and the factors affecting their attitudes.Methods: A questionnaire-based survey conducted in the campus of a university in Hong Kong. Preferences and priorities of participants on cardiopulmonary resuscitation in various situations and case scenarios, experience of death and dying, prior knowledge of DNR and basic demographic data were evaluated.Results: A total of 766 students participated in the study. There were statistically (...) significant differences in their DNR decisions in various situations between medical and non-medical students, clinical and preclinical students, and between students who had previously experienced death and dying and those who had not. A prior knowledge of DNR significantly affected DNR decision, although 66.4% of non-medical students and 18.7% of medical students had never heard of DNR. 74% of participants from both medical and non-medical fields considered the patient’s own wish as the most important factor that the healthcare team should consider when making DNR decisions. Family wishes might not be decisive on the choice of DNR.Conclusions: Students in medical and non-medical fields held different views on DNR. RationaleSurrogates of critically ill patients often have inaccurate expectations about prognosis. Yet there is little research on how intensive care unit clinicians should discuss prognosis, and existing expert opinion-based recommendations give only general guidance that has not been validated with surrogate decision makers.ObjectiveTo determine the perspectives of key stakeholders regarding how prognostic information should be conveyed in critical illness.MethodsThis was a multicenter study at three academic medical centers in California, Pennsylvania, and Washington. One hundred eighteen key stakeholders completed in-depth semistructured (...) interviews. Participants included 47 surrogates of adult patients with acute respiratory distress syndrome; 45 clinicians working in study ICUs, including physicians, nurses, social workers, and spiritual care providers; and 26 experts in health communication, decision science, ethics, family-centered care, geriatrics, healthcare disparities, palliative care, psychology, psychiatry, and critical care.Measurements and main resultsThere was broad support among surrogates for existing expert recommendations, including truthful prognostic disclosure, emotional support, tailoring the disclosure strategy to each family's needs, and checking for understanding. In addition, stakeholders offered suggestions that add specificity to existing recommendations, including: In addition to conveying prognostic estimates, clinicians should help families "see the prognosis for themselves" by showing families radiographic images and explaining the clinical significance of physical manifestations of severe disease at the bedside. Many physicians did not support using numeric estimates to convey prognosis to families, whereas many surrogates, clinicians from other disciplines, and experts believed numbers could be helpful. Clinicians should conceptualize prognostic communication as an iterative process that begins with a preliminary mention of the possibility of death early in the ICU stay and becomes more detailed as the clinical situation develops. Although prognostic information should be initially disclosed by physicians, other members of the multidisciplinary team-nurses, social workers, and spiritual care providers-should be given explicit role responsibilities to reinforce physicians' prognostications and help families process a poor prognosis emotionally.ConclusionsFamily members, clinicians, and experts identified specific communication behaviors that clinicians should use to discuss prognosis in the critical care setting. In this paper, I defend H. L. A. Hart against two prevalent criticisms of his views on social rules and the obligations with which these rules are often associated. These criticisms, I argue, rely on misunderstandings ormischaracterizations of what Hart actually intended. These misunderstandings are plausibly explained by a failure on the part of his critics to appreciate fully two of the valuable lessons Hart sought to communicate in his inaugural lecture. First, words like ‘rule’ and ‘obligation’ should not be (...) removed from their various contexts of use and subjected to abstract philosophical definition and analysis. Adequate treatment of pain is essential to alleviate suffering, yet studies show that patients with terminal or serious illness receive inadequate pain relief. In the case of terminally ill patients, adequate palliation of pain may be likely to reduce requests for physician-assisted suicide. This issue of the journal addresses barriers to effective pain relief and suggests how treatment of pain can be improved. The symposium features the Pain Relief Act, which is designed to provide practitioners who prescribe controlled substances for (...) pain with protection from inappropriate legal sanctions. En el presente artículo se presenta, desde la filosofía, primeramente la tesis de L. Zea, según el cual la universidad del hombre americano, se halla en la aceptación de la diversidad concreta de las pluriformes maneras de ser de los americanos. Europa recién ahora se pone filosóficamente el problema de la pluralidad cultural. Se analiza luego la forma de considerar el gobierno y las leyes tanto de los americanos sajones como de los americanos latinos, cuando construyen sus propias formas de (...) ser. Se considera el intento y el fracaso al querer éstos imitar a aquéllos. Trata, además, la hipótesis de L. Zea según el cual la identidad de los pueblos latinoamericanos está en no tener ninguna que lo marque como pueblo y hombre común a toda América Latina, salvo ciertos rasgos de conducta común, derivados del talante conquistador español. Ortega y Gasset prolonga este análisis y lo centra en las influencias de la inmigración en Argentina, de lo que -entre otras causas- surge una explicación para ciertas conductas que dan forma a la identidad de los argentinos. Algo no ha cambiado en el modo de ser argentino: éste no se dedica primeramente a vivir la vida, sino a hacer fortuna y defenderse. Se identifica por lo que posee o desea poseer.This article presents, from a philosophical stand, firstly Leopoldo Zea's thesis according to which the American man's universality resides in his acceptance of the concrete diversity of the various ways of being American. Only now Europe is starting to consider philosophically the problem of cultural pluralism. Secondly, we analyze the way the Saxon Americans and the Latin Americans consider government and laws during their respective processes of becoming their own selves. We examine the intent and subsequent failure of the latter at trying to imitate the former; also, Zea's thesis according to which the identity of Latin American peoples lies precisely in not having a unique identity that define them as a man and people common to all Latin America, except for some behavioral traits deriving from the Spanish conqueror temper. Ortega y Gasset extends this analysis focusing on Argentina's immigration and concluding that this partially accounts for the Argentinean behavior and identity, their priority being to make a fortune and to defend themselves. Nuestro ensayo aborda el tema de la democracia y la sexualidad en las sociedades autoritarias. La importancia del tema para la reflexión filosófica política contemporánea no dejó de ser retomado por diversidad de autores, como ha quedado confirmado desde Freud, Reich, Marcuse a Foucacult, Giddens, Beck, entre otros. Para ello nos centramos en la idea de alternabilidad democrática y la constitución de la personalidad democrática a partir de la sexualidad. A la vez, queremos en esta oportunidad hacer un homenaje a (...) W. Reich, quien se propuso a lo largo de su extensa obra escrita a principios del siglo XX, buscar una mejoría para el hombre común por medio del conocimiento de la sexualidad humana con el fin de alcanzar una vida más completa y digna. Por sus investigaciones, este autor, tuvo como respuesta la ignorancia persecutoria de vida al proponer el gozo de ser sexual como principio para el restablecimiento de una socialidad democrática a partir del factor del principio democrático de vida erótico y sexual ilícito en los humanos. The importance of the topic to contemporary political philosophy has not been overlooked by diverse authors, from Freud, Reich and Marcuse to Foucault, Giddens and Beck, among others. We focus on the idea of democratic alternation and the constitution of the democratic personality from sexuality. We also would like to pay homage to W. Reich, who intended, throughout his extensive work written in the early XX century, the improvement of ordinary man by getting to know human sexuality, reaching thereby a more complete and dignified life.
